Release of LiabilityI understand and acknowledge that receiving tattoos and piercings involves certain risks, including but not limited to infection, allergic reaction, scarring, and pain. I confirm that I do not have conditions that contraindicate such procedures (such as allergies, skin conditions, or bloodborne illnesses) unless I have disclosed these to the Artist/Studio. I release the Artist, Studio, and their agents from any claims or liabilities arising from the services provided, except in cases of gross negligence or willful misconduct.
Cancellation and Refund PolicyI acknowledge that appointments may be subject to cancellation and that fees for deposits or memberships may be non-refundable as outlined in the Studio’s policy. I agree to notify the Artist/Studio in a timely manner should I need to reschedule or cancel my appointment.
